How to fix it

  1. Update both Iris Shaders and Distant Horizons to their latest available builds for your Minecraft version so their API versions match.
  2. If you are on Iris 1.11.4 and no newer update for Iris or Distant Horizons is available yet, downgrade Iris Shaders to version 1.11.3.
  3. As a temporary workaround, turn off shaders in Options -> Video Settings -> Shader Packs (or edit config/iris.properties and set enableShaders=false), since the crash only happens when shaders are active.
